Aims & Objectives

  • Master the practical application of the entire spectrum of surgical techniques for soft tissue management around immediate and delayed implant placements, with the most modern regenerative principles in soft tissue management.
  • This course emphasises reverse tissue architecting for the optimization of implant and pontic site development to address Soft tissue reconstruction. We will discuss step by step the most common Soft tissue procedures, Emergence profile shaping and preserving, as well as the Entire Spectrum of soft tissue grafting procedures and restorative protocols.
  • It will also demonstrate the procedures with a live hands-on and allow participants to practice the most important soft tissue augmentation techniques on specialised models for soft tissue surgeries!

1- The 7 concepts of Biologically and Esthetically Stable Emergence Profile and Peri-emergence tissue complex

  • 3D implant positioning, supra-crestal complex and Vertical soft tissue thickness.
  • Platform switching, conical connections and crestal bone preservation.
  • Impact of 3D implant position on Emergence profile design and zones.
  • Preserving papillary slopes in immediacy and Creation in cases of Delayed placement.
  • Adequacy of soft tissue and keratinization.
  • Relative 3D position of the labial emergence point.
  • Sub gingival prosthetic materials for tissue adhesion and the one abutment one time concept.

3- Microsurgical Instrumentarium:

  • Microsurgical Instruments.
  • Different types of Suture Materials.

2- Fundamental Science & Soft tissue Considerations for implants in the esthetic zone

  • Anatomical and Histological Considerations for Intra-oral Soft tissue Donor Sites.
  • Avoiding INtra-operative and post-operative Donor Site bleeding.
  • Biology and Wound healing in single extractions and multiple extraction sites.
  • Anatomical considerations and blood supply of the anterior maxilla.
  • Smile analysis, Pink and white esthetics, esthetic site analysis and esthetic risk assessment.

4- Clinical cases and work flow of soft tissue Management protocols around implants placed in Healed sites and healed pontic sites.

  • Clinical Cases demonstrating the entire spectrum of clinical Scenarios.

5- Clinical cases and work flow of soft tissue Management around implants placed in Extraction sites and immediate pontic sites.

  • Clinical Cases demonstrating the entire spectrum of clinical Scenarios.

6- Video demonstrations on models and illustration maps for the entire spectrum of soft tissue harvesting techniques & closures, recipient site preparation and suture stabilising grafts, customized healing abutment and immediate provisional crowns shaping and fabrications:

Cases of sufficient soft tissues in healed sites:

  • Conventional flap design technique for delayed implant placements in cases of adequate soft tissues with simultaneous fabrication of either customized healing abutments or provisional crown for creating soft tissue contours.
  • Conventional 2nd stage surgery techniques for submerged implants in cases of adequate soft tissues with stock healing abutments or customized healing abutments or provisional crown for creating soft tissue contours in cases of delayed placements.
  • Site preparation for pontic site management.

Cases of sufficient soft tissues in immediate extraction sites:

  • Dual zone socket management with simultaneous customized healing abutment or immediate loading with provisional crown.
  • Ridge preservation and ovate pontic preparation for pontic site management.

Pedicled graft techniques:

  • Pedicled grafts: roll, modified roll, apically displaced modified roll, vip ct graft (for immediate and delayed sites) and their suture stabilizing techniques.
  • Apically positioned flap and suture stabilizing techniques.

Free graft techniques donor sites and recipient sites:

  • Free gingival graft harvesting and donor site closure technique.
  • De-epithelialized free gingival graft harvesting and donor site closure techniques.
  • Tuberosity graft and edentulous ridge free gingival graft harvesting and donor site closure techniques.
  • Sub ep. Ct graft, single incision ‘l- incision’ and trap door incision harvesting and donor site closure techniques.
  • Immediate site tunneling technique and sub ep ct graft stabilization.
  • Delayed sites flap reflection and labial partial thickness dissection to prepare recipient bed for a sub ep ct graft stabilization.
  • Sulcus deepening at recipient site and free gingival graft stabilization.
  • Principles of periosteal release coronal flap displacements at recipient site.

7- Live demo and hands on session using 'specialised models for soft tissue surgeries'

  • Harvesting free gingival grafts, donor site closure, recipient site prep by sulcus deepening and suturing techniques for stabilizing the graft.
  • Harvesting sub ep ct grafts (single incision, ‘l’ incision and trap door technique, donor site closure, & immediate extraction recipient site prep by tunneling and suturing techniques for stabilizing the graft.
  • Harvesting de-epithelialized free gingival grafts, donor site closure, & healed recipient site prep by flap reflection and partial thickness dissection and suturing techniques for stabilizing the graft.
  • Harvesting vip ct grafts, donor site closure, recipient site prep by tunneling and suturing techniques for stabilizing the graft.
  • Roll flap and modified roll flap.
  • Apically positioned flap and suture stabilizing techniques.
  • Periosteal release and coronal flap advancement.

Mukesh, Sam and Azhar have experience and training in mentoring and are ADI mentors. They have been vocational training (VT) trainers and coached countless colleagues over the years.

They, themselves have participated in lifelong learning and have been fortunate to have been trained by some of the giants of the implant field notably Professor Hilt Tatum from whom they have also absorbed and assimilated the ethics of patient interaction that underpins their approach to dentistry to this very day.

They are passionate in sharing what they have learned over their professional lives. Mukesh often talks about empowerment of the dentist. This includes sharing of all knowledge in an open, accessible, honest, unbiased and practical way that allows a dentist to expand on their skills.
